The Planning, Monitoring and Evaluation (PME) system is the approach adopted by PFC to coordinate program planning processes, implementation activities and evaluation guidelines in an integrated and coherent way. The participatory planning, monitoring and evaluation framework establishes the foundations for developing a consistent body of information on PFC’s performance. This information, in turn, contributes to informed policies, strategies and practices. This planning and accountability model includes 3 components:
- Programs and new initiatives are the result of planning activities which provide a clear statement of program goals and objectives, phased program implementation steps, and desired outcomes and performance measures.

- What do we want for our children, families and communities?
- What strategies do we think works to achieve the outcomes we want?
- How will we know if we have achieved the results we want? (PFC outputs and outcomes.
- Program implementation are monitored to ascertain the degree to which target populations are being reached, whether the delivery of services is consistent with the program planning documents, and what resources are being or have been used in service delivery. Monitoring begins as soon as the project does. Program monitoring is directed at three key questions: (1) the extent to which a program is reaching the appropriate target population; (2) whether or not the delivery of services is consistent with the program design specifications; and (3) what resources are being or have been expended in the conduct of the program.
- Program or outcome evaluation assesses the extent to which planned activities produce the desired outcome among a target population.

The PME model is led by the Planning and Evaluation Committee consisting of key representatives from the collaborating organizations Planning, Development and Communications is the staff support unit within PFC charged with assisting the organization in its efforts to improve its ability to become a self-correcting organization through planning, monitoring and evaluation efforts.
